Not just a mental record, but actively write them down and be intentional about it.

It sounds so simple, but it’s one of the most powerful tools you can have at your fingertip. 

We try to remember everything in our heads, hoping we’ll recall the details when the doctor asks. But when you’re stressed, tired, or in pain, it’s easy to forget key pieces of information.

I’ve seen it happen over and over.

A woman walks into the clinic and explains her symptoms, but because she can’t remember exactly when they started, how they changed, or what made them better or worse, it becomes harder to piece together the full picture. This can delay diagnosis and treatment.

Here are some reasons why tracking matters:

It gives your doctor clarity—you’re essentially handing them a timeline of your health. It’s like giving them a road map instead of asking them to find the way blindfolded.

It reveals patterns – Sometimes, symptoms are linked to your cycle, certain foods, stress, or lifestyle habits. You’ll spot these faster when they’re written down.

It saves you from forgetting the small but important details—that little discomfort you felt two weeks ago, what made it slightly better, or the day your symptoms suddenly worsened—these details matter.

You don’t need fancy tools—just a notebook, your phone’s notes app, or a symptom tracker template.

Some of the things to record include:

  • What started first
  • The date and time (if relevant)
  • How it progressed
  • What helped or worsened it
  • The common associating factors, like if it occurs only during your period, or when you eat a particular food, etc.
  • What you’ve done about it so far
  • Where you’re at now

Even writing just a few lines daily can make a HUGE DIFFERENCE over time.

The truth is that your doctor may only get a few minutes with you during a consultation. Your notes give them the missing context they can’t get from tests alone. 

When you’re dealing with complex conditions like PCOS, endometriosis, autoimmune disorders, or unexplained symptoms, these records can be the key to faster answers.

Here’s a classical example:

Endometriosis is a medical condition where tissue that’s similar to the lining of the uterus (the endometrium) starts growing outside the uterus.

In simple terms, the tissue found in your womb is also located in other parts of your body, like the pelvic region, intestine, or even your lungs (rare, but possible).

This means that for each month you’re menstruating, you’re also bleeding into these places.

Something simple like noting the fact that the pain or bleeding from a particular site only occurs during menstruation can do an amazing job of pointing your doctor to the possibility of endometriosis.

Basically, it’s like giving your doctor a one-page document that summarizes the important details.

Your health matters too much to leave to memory.
